Privacy

Notice ofPrivacy Practices.

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Our commitment

Frozen In Time Parlour is required by the Health Insurance Portability and Accountability Act (HIPAA) to protect the privacy of your protected health information: information that could reasonably identify you and that relates to your past, present, or future physical or mental health, the care you receive, or payment for that care.

This notice describes how we may use and disclose that information, and your rights regarding it.

How we may use and disclose your information

Treatment

We use your information to provide care. For example, we review your health history to decide whether a treatment is appropriate for you, and we document the areas and doses of injectable treatments.

Payment

We use your information to bill for services, for example by including procedure details on a receipt or working with a payment processor. Payment card data is handled by a processor that meets payment card industry security standards. We do not store card numbers.

Health care operations

We use your information for routine operations, such as appointment reminders, reviewing the quality of care, training, and running the practice.

Business associates

We share limited information with service providers who perform functions for us under written Business Associate Agreements. Each is contractually bound to protect your information to the same standard we are. These may include:

  • Our scheduling and intake services
  • Our phone, voicemail, and messaging services
  • The compounding pharmacy that prepares prescribed medication
  • Our supervising or collaborating physician
  • Professional advisors, such as attorneys and accountants, where required

Uses and disclosures that do not require your authorization

We may use or disclose your information without your authorization:

  • When required by law, such as a court order or subpoena
  • For public health activities, such as reporting adverse reactions
  • To report abuse, neglect, or domestic violence, as required by Texas law
  • For health oversight activities, such as state licensing board audits
  • For law enforcement, coroners, medical examiners, and funeral directors, where required
  • For workers' compensation claims
  • To prevent a serious threat to health or safety

Uses and disclosures that require your written authorization

Any use or disclosure not described above, including marketing, the sale of your information, and most uses of psychotherapy notes, requires your written authorization. You may revoke an authorization in writing at any time.

Before and after photographs: we never use your photograph in marketing without a separate signed photo release, which you may revoke at any time.

Your rights

You have the right to:

  • Access your medical record, and receive a copy within 30 days of a written request. A reasonable, cost-based fee may apply.
  • Ask us to amend your record if you believe it is incorrect.
  • Receive an accounting of disclosures: a list of disclosures made for purposes other than treatment, payment, or health care operations.
  • Request restrictions on how we use or disclose your information. We are not required to agree, except for disclosures to a health plan about a service you paid for in full yourself.
  • Request confidential communications, for example at a specific phone number or address.
  • Receive a paper copy of this notice on request.
  • File a complaint with us, or with the U.S. Department of Health and Human Services, Office for Civil Rights, 200 Independence Avenue SW, Washington, DC 20201, or at hhs.gov/ocr/privacy/hipaa/complaints. You will not be retaliated against for filing a complaint.
